The Case Manager (12 month contract) provides a coordinated, supportive role to individuals and families who have experienced violence and abuse, residing in our second stage shelter. Case managers are a critical piece of our multi-disciplinary team, focused on supporting clients as they walk through their individualized, healing journey.

WHAT YOU WILL BE ACCOUNTABLE FOR  

  • Primary responsibility to support clients as they identify and engage resources and referrals to increase their sense of well-being, safety, connection, and inclusion. This support involves both internal and external resourcing, shifting to meet client needs throughout an up to one year stay.
  • Provide crisis-focused, active listening, supportive safe space to meet client’s emotional needs and presenting concerns.
  • Facilitation of group and individual programming, highlighting client interest, experience in being expert in their own lives and offering opportunities to connect and reflect on what is best for themselves/their family.
  • Ensure the consistent and timely use of all necessary scales and tools that help guide ethical practice. This can include but is not limited to Service planning, acuity scales and feedback surveys.
  • Ensure Client files are accurate, factual, and upholding the dignity of each of our client’s experience. Client files are held with the utmost integrity and clarity of service, from intake through to discharge and transitions, highlighting the client experience and our role impact/intention.
  • Work in collaboration within a multi-disciplinary team, ensuring supports and interventions are client centered, flexible and coordinated.
  • Advocate alongside clients as they embrace and navigate the many systems of support for their family.
  • Collaborate and coordinate service with community agencies and partners regarding to best understand and support each client’s unique need. 
  • Establish and maintain positive, professional working relationships, both internal and external to Radiance.
  • Engage in professional reflection, with a growth mindset through informal and formal supervisions, staff meetings and larger agency events. This includes working within your scope of practice, upholding your registration body requirements, and following policies and procedures outlined by the agency.
  • Other duties as assigned by direct supervisor.

QUALIFICATIONS AND SKILLS

  • Bachelor’s degree in social work.
  • Clinical Social Worker preferred.
  • 2-3 years related field working with vulnerable people with complex needs required.
  • 2+ years case management experience is a must.
  • Extensive knowledge of community resources and programs.
  • Proven skills in de-escalation and supportive counseling with families. This includes critical/flexible thinking and strong, rapid rapport building skills.
  • Proficient in Microsoft Office Suite including Word, Excel, PowerPoint, and Teams.

JOB REQUIREMENTS

  • Clear Police Security/Vulnerable Sector Check to be maintained.
  • Clear Child Intervention Check to be maintained.
  • Membership/registration with the appropriate regulatory body under the Health Professionals Act in Alberta (e.g., ACSW) required.
  • Valid First Aid certificate including infant and child CPR.
  • ASIST Training.
  • Mon – Fri: 9am – 5pm, plus one shift of 10:30am – 6:30pm; with some weekends throughout the year.
  • Standing, bending, crouching, and lifting required.
  • This role is not eligible for Hybrid Remote work.
