Outreach Supervisor
The Supervisor provides leadership and operational oversight for the In Place program and the Older Adult Stabilization and Crisis Outreach Team (OASCOT), ensuring high-quality, client-centered services for adults 55+. This role guides staff in delivering social prescribing and crisis-stabilization supports that enhance mental health, reduce isolation, strengthen natural support networks, and divert older adults from emergency responders by connecting them to appropriate resources.
Senior Executive Assistant
The Senior Executive Assistant provides high-level administrative support to the CEO and Board, aiding strategic and operational management with a collaborative, solutions-focused approach. They also lead and supervise the administrative team, ensuring consistent, efficient, and exceptional support across the organization.
Link Worker
In collaboration with the In Place Outreach Team, the Link Worker will support participants in living independently within the community and improving their social determinants of health. Under the direction of the Supervisor, the Link Worker will use best practices in social prescribing to connect participants with social, community, and partner supports, enhancing their health and well-being and assisting them in safely aging in place.
