The Outreach Supervisor leads two Outreach programs supporting adults aged 55+: In Place Social Prescribing, which builds connection and reduces isolation, and the Older Adult Stabilization and Crisis Outreach Team (OASCOT), which provides coordinated support to older adults facing elevated risk. The Supervisor oversees daily operations, coaches staff, provides case consultation, and monitors service quality and outcomes to ensure responsive, participant-centred services.
