The Program – {Live-in Programs}

The Position – {Community Connector}

Enviros currently has an exciting opportunity for a Community Connector who will use a Wraparound approach to work collaboratively with Children Services to provide support for children and youth in our 3 live-in programs.

The Community Connector will be responsible for working from a client centred, strength-based approach in a collaborative team environment with staff from the live-in programs, Children Services and Natural Support members. The Community Connector will provide a structured, creative and individualized team planning process which supports children and youth to achieve individualized goals that promote crisis stabilization, lifelong connections and successful transition/outcomes.

Responsibilities

· Engaging and interviewing youth on strength, needs and culture to be compiled in youth presentations

· Identifying, engaging, strengthening and expanding community and natural support networks

· With a team approach, developing integrated plans that address priority needs of the individual with external stakeholders

· Work with the live-in teams to support action steps and coordinated efforts

· Facilitating a process for input regarding comprehensive crisis stabilization and prevention safety plans

· Facilitating and coordinating team members from various systems

· Developing comprehensive transition plans

Competencies

· Awareness of appropriate community resources and services

· Strong writing, organizational, program planning, facilitation and interpersonal skills

· Prior facilitation skills an asset

· The Community Connector must have a demonstrated ability to work independently as well as within an integrated, multi-disciplinary team

Education and Experience

· Diploma/Degree in Human Services

· 5 years experience in the field of Human Services

· Knowledge and experience working with families

· Knowledge of addictions and mental health

· Current standard First Aid Certification

· Police check and Child Intervention Record Check (current within six months)

· Valid Driver’s license and adequate insurance coverage

Other Qualifications

· Valid Class 5 driver’s license (Class 5 GDL will not be accepted)

· Own transportation

· The successful applicant will be required to work flexible hours to accommodate client meetings.
