Summary

With a trauma informed lens and client centered approach, the Case Manager will facilitate, through a coordinated case management process, the moving of men and their families from their current crisis situation into our interim shelter and then to more permanent-housing solutions, with supports.

Description

Key Responsibilities and Deliverables:

  1. Working on a case management basis, the Case Manager will screen new referrals to the family shelter, assess for program suitability, prioritize new intakes into the family shelter, and provide ongoing in house supports or community referrals.
  2. Engage in preliminary goal setting/care planning to assess program participant’s current conditions, determine housing history, barriers to future housing, and plans for future housing. 
  3. Provide crisis intervention as needed.
  4. Coach and support program participants as needed to access appropriate services through CCMF or external agencies.
  5. Monitor and follow up on program participants with regular case management meetings. 
  6. Adhere to all CCMF policies, procedures, rules, and guidelines for the safety and security of program participants and staff/volunteers.
  7. Maintain contact with program participants for the purpose of:
    • monitoring progress of their goal plans
    • following established procedures, referring to appropriate CCMF and community based resources and
    • advocating on behalf of the program participant to other CCMF programs or outside resources as appropriate
  8. Attend case management meetings with outside agencies to support mutual clients and attend interagency meetings that may benefit family shelter clients or their children.
  9. Assist the Family Housing & Programs Manager or other staff/volunteers with room checks.
  10. Implement appropriate emergency procedures as necessary.
  11. Maintain accurate records and documentation as required (e.g. communication log notes, case notes, room check notes, goal planning records, and incident reports).
  12. Answering phone calls from the CCMF Alberta phone line.
  13. Conducts research to enhance service delivery and inform community planning initiatives (i.e. survey development, program assessment, community assessments).
  14. Consults / engages with community members, CCMF staff, and other stakeholders to identify community priorities and inform planning.
  15. Perform additional duties when required (e.g. assist in preparing rooms for new program participants, moving furniture, updating inventories, etc.).

Qualifications:

  • relevant University or Community College education in social work; social sciences; addictions case management, or related fields
  • minimum 1-year experience working in a social services setting
  • strong interpersonal skills
  • experience in working cooperatively with community resource agencies
  • effective communication skills, particularly in negotiation and advocating for program participants’ needs
  • knowledge of local housing placement resources and other social service providers is an asset
  • good writing skills for documentation and correspondence
  • strong computer skills, including a working knowledge of Google Workspace and Microsoft Office
  • trained in Standard First Aid/CPR, ASIST, and Non-Violent Crisis Intervention or equivalent trainings

Successful candidate will be required to provide upon hiring:

  • A clear criminal record with vulnerable sector screening

Service Responsibilities of the Contractor

  • Report directly to the Family Housing & Programs Manager
    • Weekly update meetings
    • Preparation of monthly reports as needed
  • Assist monitoring the housing email address, including:
    • Voicemail emails are monitored regularly and forwarded to the appropriate team member, as necessary.
    • Collaborating with the National Intake Team on referral inquiries
  • Program Administration
    • Engage with internal and external referral sources to provide continuity of care and increase program participant engagement.
    • Communicate with CCMF counselling team members and make appropriate referrals to other CCMF programs where appropriate.
    • Support program participants in scheduling and engagement in CCMF programming.
    • Contributes ideas for ongoing improvement and effective delivery of high quality, well-coordinated programs and services at CCMF Alberta.

As a representative of CCMF Alberta, the Case Manager will:

  3. Understand and act in accordance with the Policies, Standards, and Procedures of CCMF.
  4. Develop and maintain respectful, cooperative working relationships that contribute to the integrated, seamless delivery of services to all program participants of CCMF Alberta (men, children, families, and communities).
  5. Ensure accuracy, confidentiality, and safekeeping of agency records.
  6. Know the relationship of the services among local CCMF Alberta programs and other regional or national CCMF programs to the organization’s Mission and strategic direction.
  7. Participate in regular team meetings as required.
  8. Refer program participants to other CCMF Alberta programs where appropriate.

Responsibilities of CCMF Alberta

As a responsible contractor of service providers, CCMF Alberta shall:

  1. Provide access to general office equipment, internet, and telephone, as required.
  2. Promote and grow the programs though internal referrals, networking, and outreach to referral partners and in the broader community.
  3. Clearly communicate expectations about job duties and work tasks assigned to the Contractor.
  4. Provide a healthy, respectful, supportive, and harassment-free environment for all personnel and program participants.
