Description

CCIS’s Resettlement and Integration Services (RIS) Division is looking for a new CSS Case Manager and is accepting applications from qualified job seekers. The Intensive Case Manager provides comprehensive support to newly arrived government-assisted refugees. This position offers intensive case management services tailored to the unique needs of each client, including regular check-ins, targeted referrals, and follow-up support.

This position seeks to empower refugees, facilitate their settlement into the community, and support their overall well-being during resettlement. This position is responsible for managing a caseload of individuals and families for two years.

Individuals in this role will apply GBA Plus training to identify and understand the specific barriers that clients may face based on their intersecting identity factors. With this approach, they will ensure that the needs of all clients (including newcomer women, high-needs refugees, newcomers with disabilities, and LGBTQI+ newcomers) are met through targeted and responsive services and supports.

Roles and Responsibilities

 

  • Provide intensive case management services tailored to the complexity of the client’s needs, including coordinating and monitoring the delivery of services.
  • In collaboration with client and/or family unit, conduct comprehensive needs assessments and develop individualized settlement plans by using nationally developed standardized triage tools. Ensure that the settlement plan and client goals are client-centred, achievable and measurable for clients to access settlement services on their own.
  • Conduct timely, professional interventions to ensure responsive, appropriate, and client-centred service delivery.
  • Participate in supervision and consultation regularly with the CSSP manager.
  • Coordinate and monitor the delivery of services, ensuring timely access to healthcare, housing, education, language training, and social support programs.
  • Provide emotional support, crisis intervention, and advocacy to clients facing complex challenges during their resettlement journey.
  • Assist clients in navigating systems and services, addressing barriers, and accessing appropriate resources.
  • Maintain regular contact with clients through frequent check-ins at their homes, assess their progress, address emerging needs, and provide ongoing support.
  • Conduct home visits, phone calls, and in-person meetings to ensure clients' well-being, monitor their settlement goals, and identify any potential service challenges or gaps.
  • Make appropriate referrals to external service providers, such as healthcare professionals, mental health counsellors, educational institutions, and employment agencies. Follow up on the progress of referrals to ensure clients receive the necessary support.
  • Provide interpretation services during appointments, meetings, and interactions with various service providers to ensure effective communication between GARs and the community. Act as a cultural broker, facilitate understanding and promote accurate information exchange by accompanying clients to various community appointments.
  • Assist clients in developing essential life skills, such as budgeting, navigating transportation, accessing community services, and understanding Canadian laws and regulations.
  • Monitor and evaluate clients' progress, documenting milestones, challenges, and outcomes.
  • Advocate on behalf of clients, with government, external organizations, public systems, and the broader community.
  • Maintain accurate and confidential case files, documenting all client interactions, interventions, and progress. Regularly report service delivery in the designated agency and funder database
  • Participates in professional development training, team meetings, and meetings in the community, when required.
  • Follows CCIS policies and procedures, and performs job duties according to the agency's health and safety policies
  • Any other tasks needed by the supervisor, manager and RIS Director.

Qualifications:

  • Bachelor's degree in social work, Psychology, Counselling, Human Services, or experience in a related field or a combination of education and experience
  • AAISA certification at level three is an asset
  • At least three years of experience working either in resettlement or in the settlement sector and/or case management, social work, and/or related field.
  • Knowledge of evidence-based models and best practices for case management and service delivery in settlement and resettlement contexts.
  • Three years of experience working with refugees, immigrants, or vulnerable populations.
  • Strong understanding of the challenges faced by refugees and the ability to provide culturally sensitive services.
  • Demonstrated ability to use strengths-based, client-centred, and trauma-informed approaches to service delivery.
  • Knowledge of local immigrant-serving agencies, their programs and services, current immigration issues, and resources available in the community.
  • Adaptable to changes in refugee pattern arrivals and the ability to quickly respond to a fast-paced environment
  • Non-violent crisis intervention and First Aid/CPR training are assets.
  • Knowledge of a second language is an asset.
  • High level of interpersonal skills to manage conflict and work effectively with a diverse group of clients, staff, and community members.
  • Strong analytical, problem-solving, and decision-making skills
  • Attention to detail in data reporting, establishing priorities, meeting deadlines, and monitoring follow-up with clients by staff.
  • Ability to multitask and work in a fast-paced, multidisciplinary, culturally diverse environment.
  • Must be someone who has a positive approach working in the field and outside of a traditional office setting
  • Must be someone who has a positive attitude, is flexible, reliable, proactive, collaborative and takes initiative
  • Proficiency in using computer systems, databases, and office software for case management and documentation purposes.
  • Available to work irregular hours, including evenings and weekends, when needed.
  • Must be legally eligible to work in Canada
