Position Overview
Inn from the Cold (The INN) is seeking passionate and committed Outreach & Housing Case Managers to support families in crisis through the Homeless Emergency Response (HER) Project. As part of this one-year pilot initiative, Case Managers will support up to 20 families as they transition from homelessness to stable housing, utilizing a hotel-stay model for temporary shelter. The HER Project expands short-term emergency housing options by partnering with local hotels when The INN’s family shelter reaches full capacity.

Case Managers will report to the HER Program Manager and collaborate with Family Support Workers, applying a trauma-informed and housing-focused approach to help families achieve stability, find housing more quickly and prevent future homelessness. Key responsibilities include developing individualized housing plans, assisting with
housing searches, working closely with the INN Housing Specialist and Prevention & Diversion team, providing and connecting families with essential resources, including financial support.

What You’ll Do

  • Engage and develop rapport with families in a trauma-informed and culturally sensitive manner.
  • Collaborate and coordinate with the HER project team members to support families, completing tasks together, coordinating activities, and responding to family needs.
  • Establish contact and meet a family in person within 24 hours of their hotel arrival, and complete the Intake package including participation agreements, consents and ROIs.
  • Coordinate the family’s immediate basic needs including food support (e.g. gift cards), clothing/hygiene items, transportation and health support with Family Support Workers.
  • Monitor family stabilization and coordinate with the INN's Shelter team if a family's situation changes and requires them to be transferred to the Shelter.
  • Respond to urgent/critical family situations, working collaboratively with the HER Project team and hotel staff as necessary to ensure support and resolution.
  • Meet regularly with families to provide support, develop goals and monitor progress toward stable housing.
  • Evaluate family needs, risks, and strengths to develop and implement a family case plan that prioritizes mutually agreed upon stable and permanent housing solutions.
  • Assess the family's financial assistance needs and provide support in establishing initial income.
  • Assist families with housing searches, online listings, landlord communications, and attending housing viewings, attend rental walk-throughs and lease signings (when possible).
  • Connect with landlords and other community agencies to advocate for the family's needs and facilitate financial support payment processes.
  • Support clients with initial housing setup, moving support and essential furniture and household items.
  • Work collaboratively with the INN’s Housing Specialist, Prevention & Diversion (P&D) program, and other community P&D programs to support rapid re-housing efforts.
  • Maintain regular communication with the hotel partners to ensure smooth coordination of shelter services.
  • Work with larger INN team and community resources to leverage internal strengths and resources and gather external resources to support families transitioning from their hotel stays to community.
  • Assess, document and address potential safety situations/critical incidents within the project to ensure proper safety and emergency protocols are used.
  • Maintain accurate, timely, and confidential documentation of family interactions, case notes, intake and assessment information, safety concerns and records of support and service referrals.
  • Assist in tracking family progress and hotel stay data to contribute to program evaluation and service improvement.

What You Bring to Our Mission

  • A deep commitment to supporting at-risk and vulnerable families, with a strong desire to make a meaningful impact in the homeless-serving sector.
  • Completion of a diploma or degree program related to social work, family case management, or human services is preferred but a combination of education, experience and training will be considered.
  • 2 years’ experience in family case management, family support and/or crisis support.
  • Experience working with vulnerable populations experiencing homelessness.
  • Knowledge of trauma-informed and client-centered care, and conflict resolution skills.
  • Knowledge and understanding of issues related to homelessness and its impact on families
  • Ability to respond to the complex needs of families in a family shelter setting.
  • Ability to plan housing-focused case plans effectively, monitor progress and assist family with goal completion.
  • Knowledge of housing-focused community resources, services and supports.
  • Commitment to working in a team environment and collaborating with other program teams.
  • Strong communication, collaboration, problem-solving and documentation skills.
  • Strong work ethic, sound judgment, and reasoning skills, along with a commitment to discretion and family and staff confidentiality.
  • Ability to effectively manage multiple competing priorities and timelines.
  • Ability to deal with people sensitively, tactfully, diplomatically, and professionally
  • Computer literacy, including knowledge of Microsoft Office 365 including Teams and Outlook.
  • Proficient documentation including case notes and case plans, and survey reporting skills using a client information management system.
  • Current First Aid/CPR is required, and Suicide Intervention Certification/ASIST is an asset.
  • A valid driver’s license and reliable car is required.
  • Police checks and Child Intervention checks are required for employment.
  • Availability to work weekdays and either Saturday or Sunday shift for 7 day a week support.
