In collaboration with the In Place Social Prescribing Outreach Team, the Link Worker will support participants in living independently within the community and improving their social determinants of health. Under the direction of the Supervisor, the Link Worker will use best practices in social prescribing to connect participants with social, community, and partner supports, enhancing their health and well-being and assisting them in safely aging in place.
